Medical Home

 

 
Visit checklist for Medical Home Patients:
  1. Please update the "Medical Home Patient" clinical overview as needed on the day of the visit. Additionally, write your plan in the area called "Summary of this visit's plan" so that at the next visit, we can remember what we did for the family previously, and what we need to follow up. 

  2. For routine follow up visits and well child checks, please update the section regarding the family's short and long term goals. 

  3. For your actual progress note, remember to use the ".PCCmedhome" note template. When you use this special template, you will notice the Assessment and Plan area has an "auto-update" section. This area is linked to the "Medical Home Patient" entry in the Problem List. Any changes that you make to that entry will automatically get pulled into your note template. This essentially becomes your assessment and plan. This is done to help you avoid double work. 

  4. Do not use "medical home patient" as the primary visit diagnosis! This means nothing to the insurance company and CCS so it will not approve any referrals based on this diagnosis. Use the actual medical diagnosis (e.g., asthma, cerebral palsy, etc) to associate orders and referrals in CareConnect.

  5. Prepare an "AVS" (after visit summary) under Patient Instructions. Please use the template ".PCCmedhomeavs" and fill out the sections as needed to remind the family what we will do for them and what they need to do for themselves. There is a Spanish version.  

 

For families who prefer Spanish as their primary language, please use one of the medical language interpreter services available in the clinic (computer, ipad, phone, in-person, etc).
