  • As you prepare to complete your pediatrics residency, there are a few "housekeeping" items you must do to check out from UCLA/SM, Miller, Olive View, and Cedars. See below for instructions and forms. 

  • Note: Do not finalize check out (ie. turn in badges, etc.) if you still have back up shifts! 

  • Watch the Resident Check Out Process Video! (10 min) The video is still applicable despite being made in 2021. Please reach out to Cherie Lewis (not Megan Fuller) if you have any questions.

 
 
 
 
 
 
UCLA Check Out - June 2025

 

As part of UCLA’s check-out process, you must complete a checkout meeting (~15 mins) with Dr. Alan Chin at which time you go over your procedure logs and electives. Click here to schedule your virtual Zoom meeting. Feel free to schedule your meeting now while you are working on completing the check out process.

Once you are ready to drop off your check off form, ID badges, SM parking pass, and pick up your graduation certificate, please schedule a time to stop by the Med Ed office. Click here to schedule your drop off time. Please note: these need to be scheduled at least 24 hours in advance. Please let Cherie Lewis know if none of the dates/times work for you and we can see what we can do.

Miller Check out
  • Your Miller ID Badge and Meal Card should be returned to the Millers GME office . If you don't have plans to return to Millers, you may drop it off with MedEd in 3108 RRUMC.

  • You are required to receive Medical Records Clearance from Veronica Le. Please email her this completed form to confirm you are cleared (VLe11@memorialcare.org)

Olive View Check Out
  • Olive View Check Out FormRequired for all residents, including Rising Chiefs

  • Please return form, badge and parking pass directly to Samantha Donis. If you will not be returning to Olive View, you can also return these items to the UCLA MedEd office (3108 RRUMC). 

  • Current Chiefs: You do not need to complete a form. Please turn in your OV badge to Adrianna Gonzales and email her stating when your last day will be. Email: adgonzales@dhs.lacounty.gov

  • Note: If any item has been lost (ex ID badge, parking badge), the fee required needs to be included. If by check, make payable to OVMC. Cash is also accepted. Residents will not receive their graduation certificate until all the items are received or paid for.

Cedars-Sinai Check Out
  • Cedars-Sinai Check Out Form: Only applicable to those on this list

    • The linen department has very unusual hours: M-F 7am-10am and 1pm-4pm

    • All sections MUST be signed. Completed form needs to be turned into our office (3108 RRUMC) with your badge. We will mail Cedars your items.

    • For any questions regarding the Cedars check-out form/process, please contact Calvin Lee (Calvin.Lee2@cshs.org).
